   Fill out the Dealer Application Form, and mail or fax the form in along with the following.

q       Copy of your state resale certificate

q       Resale tax form (CA only) - you will be charged sales tax until the form is received

q       A picture of your place of business or a copy of your company's yellow page ad.

q       A copy of letterhead or business card
